After submitting your information you will receive a copy of your puppy’s registration certificate and birth certificate by e-mail. 

*Please note, Once the information is verified, Applications may take up to 5-7 business days.

If you haven’t received your documents after registering your dog and it’s been after 5-7 business days since you registered, then send us an email with the form below and let us know the issue along with the microchip number that originally came with the puppy.

1. Go to FreePetChipRegistry.com and click on the “Create an Account” button. It’s at the top of the page.
2. Create Account: Fill out the required fields and click on the “Register Button” at the bottom of the page. (Make sure to make a record of your username and password!)
3. Add Your Pet (Fill out the required fields and click on the “Add Pet” button at the bottom of the page and you are done!)

PROOF OF OWNERSHIP

What would happen if your dog was lost or stolen, and you had to provide proof of ownership to get him back?
